Questionnaires
To create a questionnaire, navigate to Verification Forms, either select an existing form or create a new one, then manage steps and choose the Questionnaire option.

Enter your question in the ‘Title’ field.

Provide additional clarification in the ‘Description’ field if needed for the applicant.

Choose the type of question:
Free Answer: Applicants type their own responses.
Multiple Choice: Applicants can pick one or more answers from a list.
Single Answer: Applicants choose just one answer from the options.
Dropdown: Applicants select one option from a drop-down menu.
For Multiple Choice, Single Answer, and Dropdown options, define a list of choices from which the applicant must select one or multiple options. Set both the Value and Options fields.

In the Value field: enter the internal ID for system processing.

In the Option field: enter the label that will be visible for applicants to choose from.

Free Answer

The Placeholder title field provides an example of what the user should enter.

At the Regular expression field define specific rules or patterns for the input that applicants must follow (e.g., +480001112233 for a 12-digit phone number)

At Validation Text field enter a clear error message to guide the applicant when their input doesn't match the required format.

Note: Ensure that your workflow includes the Document step and is valid. For any additional checks on an existing applicant, ensure the Applicant ID is added when creating the link.
